Class 2 and Class 1 (required for commercial licence) medicals.
Under which we have :
1) Initial: It is done when a person with a valid Class 2 medical applies for the first time for a Class 1 Medical.
2) Re-Initial: in case the last medical date (Class 1) is more than 2 years old, a fresh initial (Re-initial) Class 1 medical is to be conducted only at Air Force Medical Boarding centers, along with approved NOC by Medical Cell, DGCA.
3) Renewal: The renewal medical may be done anytime from one month before date of expiry of validity of Medical Assessment to the date of expiry of validity. If done earlier or later than that, an NOC is required.
4) Special: Any temporary unfitness or recovery from a certain illness you'll have to go through special medicals.

Class 1 Medical Assessments are valid for one year for persons less than 60 years of age in Multi crew commercial Air Transport Operations or six months for persons above 40 yrs of age in Single crew commercial air transport operations.
Class 1 medical renewals may be done after expiry of validity of Medical Assessment, but not beyond two years from the date of last medical along with NOC.
For Re-initial /delayed/ early/ Post TU or Special Medicals only, kindly obtain an NOC from Medical Dte at DGCA, New Delhi. The NOC form available in Procedure & training Manual, available on DGCA websites Annexure ‘D’ to be filled completely. Applications for NOC are to be made after obtaining confirmed appointment and to be sent along with DGCA issued Last Valid Medical Assessment on medicalnocdgca@gmail.com

YES, if an appointment at a Class I doctor is preferred, the appointment must be taken 3 months in advance.
NO, medical dates are planned and updated months in advance. While IAF Stations do not entertain any rescheduling requests, DGCA Empaneled Class 1 Medical Examiners may still offer flexibility.
The examination fees charged by the IAF medical establishments is Rs 2000/-. (Fee to be paid vide https://bharatkosh.gov.in//) However, the DGCA Empaneled Class 1 Medical Examiners may charge differently. Pilots to contact the individual doctors for further details.
